# Pelvic Floor Physical Therapy: Treat the Whole Body, Not Just Symptoms

Leaking urine. Pelvic organ prolapse. Painful intimacy. Back pain. Urinary frequency. Constipation. Pelvic pain.

These symptoms can all involve the pelvic floor but that does not necessarily mean the pelvic floor is the root cause of the problem.

At Advanced Pelvic Health Solutions, we believe the pelvic floor should be evaluated as part of a connected system rather than treated as an isolated muscle group.

### The pelvic floor does not work alone.

Your pelvic floor works together with your:

- Abdominal muscles and core
- Diaphragm and breathing mechanics
- Hips and pelvis
- Low back
- Spine
- Posture and movement patterns
- Muscles and connective tissues throughout the body
- Pressure-management system

When one part of this system is not functioning properly, the pelvic floor may have to compensate.

Sometimes, the pelvic floor is the source of the problem. Sometimes, it is responding to another problem.

And sometimes, it is the victim of a problem happening somewhere else. That distinction can completely change how treatment should be approached.

## Treating the Symptom vs. Understanding the System

Traditional pelvic floor treatment may focus heavily on the symptom itself.

For example, a woman experiencing urinary leakage may immediately be told:

“You need to strengthen your pelvic floor. Start doing Kegels.”

Pelvic floor muscle training is an evidence-based treatment for many women with urinary incontinence, and clinical guidelines recommend supervised pelvic floor muscle training for stress and mixed urinary incontinence.

But strengthening is not automatically the answer for every patient.

 

Before prescribing an exercise, we need to understand why the leakage is happening.

 

*Is the pelvic floor weak?*

*Is it unable to coordinate with the abdominal muscles?*

*Is there excessive pressure being generated during movement?*

*Is breathing contributing to poor pressure management?*

*Is there a hip, pelvic, spinal, or movement dysfunction affecting how the pelvic floor responds?*

*Is the pelvic floor actually too tense and unable to relax appropriately?*

 

These questions matter.

 

Because the same symptom can have very different contributing factors in different women.

 

# Think About a Tube of Toothpaste

Imagine squeezing a tube of toothpaste. The toothpaste is being pushed toward the opening, creating pressure. You put the cap on but the pressure inside the tube has not changed.

If you keep squeezing the tube, simply tightening the cap may not solve the underlying problem.

You have to ask:

*Why is there so much pressure in the first place?*

The pelvic floor can sometimes be approached in a similar way.

Rather than simply telling the pelvic floor to “work harder,” we want to understand what is creating the demand placed on it.

That is why our approach at Advanced Pelvic Health Solutions begins with assessment—not a predetermined list of exercises.

 

# We Don’t Treat Every Pelvic Floor the Same Way

A woman experiencing urinary leakage may need strengthening.

Another woman may need improved coordination.

Another may need better relaxation.

Another may have a movement or musculoskeletal problem that is contributing to excessive pressure on the pelvic floor.

And another may have several factors occurring simultaneously.

Research supports pelvic floor muscle training for urinary incontinence, but evidence also shows that pelvic floor conditions are complex and that different presentations require different treatment approaches.

For pelvic pain and painful intimacy, physical therapy may also address myofascial and musculoskeletal contributors rather than focusing exclusively on pelvic floor strengthening.

This is why assessment comes before prescription.

# What About Back Pain, Constipation, Urinary Frequency and Painful Intimacy?

The pelvic floor is involved in much more than continence.

Pelvic floor dysfunction can be associated with symptoms including:

### Urinary leakage

The pelvic floor must coordinate with the rest of the pressure-management system during coughing, sneezing, running, lifting and other activities.

### Urinary frequency and urgency

Bladder symptoms can involve multiple factors, and pelvic floor rehabilitation may be one component of treatment depending on the individual’s presentation.

### Pelvic organ prolapse

Pelvic floor muscle training is one conservative treatment option for some women with symptomatic prolapse, particularly earlier-stage prolapse.

### Painful intimacy

Pain with intercourse can involve pelvic floor overactivity, myofascial restrictions, pain sensitization and other physical contributors. Physical therapy approaches can include individualized exercise and manual therapy.

### Chronic constipation

The pelvic floor must coordinate with the abdominal wall and breathing mechanics to appropriately manage pressure and facilitate bowel emptying.

### Low back and pelvic pain

The pelvic floor is part of a larger lumbopelvic system. Chronic pelvic pain management may include physical therapy addressing myofascial and musculoskeletal factors, as well as movement, breathing and other contributing factors.

The goal is not to assume that every symptom comes from the pelvic floor. The goal is to discover what is actually contributing to YOUR symptoms.

 

# This Is Where One-on-One Care Makes a Difference

Your first visit should be about more than simply identifying whether your pelvic floor is “weak.”

We take the time to understand:

*What are you experiencing?*

*When did it start?*

*What makes it better or worse?*

*What activities are you avoiding?*

*What have you already tried?*

What is your body doing when your symptoms occur?

 

And most importantly:

### What do you actually want to get back to doing?

Maybe your goal is to:

- Run without leaking
- Lift weights confidently
- Jump without pain or pressure
- Exercise without pelvic symptoms
- Return to intimacy without pain
- Go through your day without constantly looking for a bathroom
- Have comfortable bowel movements
- Feel confident in your body again

Your symptoms are only part of the story. Your goals are the other part.

- Pain during intimacy.
- Pelvic pressure.
- Back pain.
- Constipation.
- Urinary urgency or frequency.
- Difficulty returning to exercise after pregnancy.

 

These symptoms may be common—but common does not mean you have to accept them as your normal.
